Just weeks after a fatal ride failure at the Ohio State Fair, an incident at the Fayette County Fair has visitors shaken up.

Jessica Waite and her 3- and 4-year-old daughters were on the Monster Truck Ride at the fair when the ride suddenly broke down.

"Her knee slammed against the side. My other daughter was in front of me and we all just kind of slammed around a little bit," Waite said. "I had no idea what happened until I got off the ride."

Photos provided by Waite show that a wheel came off on one of the trucks. Waite said other trucks ran the wheel over, throwing passengers around on the ride.

According to inspection records for the fair, the Monster Truck Ride was inspected on July 27, which was the day the fair opened. That is all the state requires for fair rides.

Channel 11 contacted state officials about the photos, and they said the incident was never reported. Officials stressed that state law only requires organizers to report accidents that involve "serious injury, illness or death."

After Channel 11 notified them, officials at the Pennsylvania Department of Agriculture said they would investigate.

A manager at the Fayette County Fair said the fair does not operate the rides, which are contracted out to a national company.

"It just could have (been) prevented very easily if they had just checked the rides thoroughly before -- you know, every day or every few hours or something. If they would make sure all the loose wheels are tightened on right or something," Waite said. "I shouldn't have to worry if this ride is going to hurt my children."
